  • The Social Security Administration will send you SSA-3373, the Function Report, after you apply for benefits.
  • It is also commonly referred to as the “Activities of Daily Living” or ADL questionnaire.
  • This Disability questionnaire is designed to gauge the severity of your condition and how it affects your daily life.
  • Some questions are personal, such as your ability to use the bathroom, bathe and dress.
  • Other questions deal with your ability prepare meals, do chores, pay bills and take care of others.

QUICK TIP:

Pay special attention to Question #6 on the SSA-3373.

The sixth question on the Function Report asks you to “Describe what you do from the time you wake up until going to bed.” Be sure to answer this question honestly. Many people feel guilty that they do not do more throughout the day so they downplay their condition and exaggerate their activities.

Do not do this. If you mainly stay in bed and watch television or read because you are physically unable to do otherwise, then say that. The point of the Function Report is to determine how your condition affects your day-to-day life. You can use the “Remarks” section to provide additional information if you run out of space.
